OK, I've been away from the sim for a while. I see no one has solved this problem in the meantime. I had another go and have found a method that enables correct calibration, while using F2/F3 to control reverse. I realised that moving the hardware throttle so that the levers were in the visual detents was the problem. You need to set your hardware throttle to the position you want the detent to be, and select that as the calibration point. So the process is as follows.
On the calibration page, select the F2/F3 option ON and leave SMART IDLE and MANUAL OFF.
Move your hardware throttle to idle and select CURRENT THROTTLE IS AT IDLE
Move your hardware throttle to approx halfway between idle and max and select CURRENT THROTTLE IS CL
Move your hardware throttle to roughly halfway between your chosen CL position and full forward and select CURRENT THROTTLE IS MCT
Move the hardware throttle fully forward and select CURRENT THROTTLE IS TOGA
Ignore where the simulated levers while you do this. Then verify the throttles move as expected in the sim and save the settings. I put CL at about halfway from idle to full, but if you prefer more manual range you can choose the detent positions that feel best for you.
